Below you see a list of forms that you can download.  All new patients should fill out all the forms.  The patient form allows us demographical and insurance information.  Remember to still bring your card to the visit.  The HIPAA form shows that you are aware that your child’s medical record is confidential.  The authorization of care is to be filled out to allow another member of your family or friend to bring your child if you are unable to come.  Lastly, the medical records release, will allow your former doctor to send the records to us or allow us to send the records where you request them to be sent.
